Volunteer Driver Overview

MTC Mentors are required to complete the MVLA Volunteer Driver Application. Simply collect and submit the materials detailed below.

Required Materials

Please collect the following items and email them to” Attn: Daisy, MVLA Volunteer Office” at mvla.volunteers@mvla.net.

  1. Obtain a current (within 30 days) DMV Driver Record Report. This report may be requested from the DMV website for a fee of $2.00 using the following link: Online Driver Record Request  (a $2.00 fee applies). You must not have any points on your license for the previous 3 years.
  2. Copy (or photograph) your current driver’s license.
  3. Copy (or photograph) of the page of your auto insurance policy that shows your name, the policy number, and verification of current coverage amounts (minimum required is $100,000/$300,000). Please note, a copy of the insurance card is not sufficient.
