PROOF OF COVID VACCINATION
All MTC volunteers must show proof of COVID vaccination. A photo of your vaccination card or a screen shot of your digital certificate will suffice. Please send to firstname.lastname@example.org and Upload to the district website here. You will need to have a google account to upload to the district website — If you don’t have one, the form will allow you to create one. Please make sure to BOTH upload to the district website AND send to the programs email address. MTC does not have access to the district website.
All MTC volunteers must provide MTC and the MVLA High School District with proof of a negative TB Test by a licensed health care provider within the past four years or a waiver stating they should not need a test.
If you don’t already have a recent test, you may obtain a TB test without cost by completing and printing this form and testing at the district’s provider, Concerta: MVLA TB Test Authorization Form. Cedar Crest Nursing and Rehabilitation Center also offers a free test for MTC volunteers (see instructions below). Lastly, your physician may complete the CTCA Risk Assessment Form if they feel you should not need to take the test. Often, your physician can complete this form without you having to make an appointment.
Please send the results to MTC at: email@example.com. You will also need this to complete the fingerprinting step for the district. Again, please make sure to do both: email to MTC AND include in your fingerprinting step for the district.
To have your test taken at Cedar Crest Nursing and Rehabilitation Center, please walk in any time between Monday through Friday between 10 am and 6 pm.

72 hours from your appointment, return to Cedar Crest to have your skin test read. No appointment is necessary. They will give you your TB Test Screening Results.
